Specializing in Interventional Pain Management; using the latest techniques, including fluoroscopy-guided epidurals, transforaminal epidural steroid injections, facet injections, caudal blocks, SI joint injections, radiofrequency ablation, and spinal cord stimulator placements, and subcutaneous discectomy.
Peter A. Kechejian, MD, CPE is a board-certified Pain Management Specialist and Certified Pain Educator. He graduated from Georgetown University School of Medicine and completed his residency in Anesthesiology and fellowship in Pain Management at the University of Massachusetts Medical Center. Dr. Kechejian has been caring for patients across Long Island for the past 25 years.
He served as the Director of Pain Management for the Anesthesia Residency Program at Nassau University Medical Center, where he thrived on teaching physicians in training different anesthesia blocks for surgery and pain procedures for chronic pain sufferers, such as epidural steroid and spine joint injections. As his love for teaching deepened, Dr. Kechejian was appointed Clinical Assistant Professor of Anesthesiology at SUNY Stony Brook University Hospital.
Subsequently, Dr. Kechejian pursued his passion for pain management at Syosset Hospital where he perfected his interventional pain management skills. Dr. Kechejian focused on providing safe, minimally-invasive outpatient techniques for treatment of chronic pain conditions, including neck and back pain, while at the same time, trying to minimize opioid therapy.
It was during his work in a private pain management practice in the towns of Smithtown and Shirley that Dr. Kechejian continued to advance his skills in spinal cord stimulation therapy for patients who remained in chronic pain despite aggressive medical and surgical treatments. Dr. Kechejian is dedicated to playing an active role in minimizing the opioid crisis in America, particularly among pain sufferers living on Long Island.
As a provider at Total Orthopedics and Sports Medicine, Dr. Kechejian is dedicated to healing patients suffering from acute and chronic spine pain conditions and syndromes. He prides himself on maintaining a patient-centered approach to pain management and creating a tailored treatment plan for his patients’ unique needs. Dr. Kechejian believes that the key to helping patients feel pain relief is to integrate the judicious use of medications, periodic pain procedures such as epidural steroid and spine joint injections, and spine surgery when needed. Through the use of multidisciplinary, integrative pain care, Dr. Kechejian works closely with the physical therapists and spine surgeons in the group to ensure that his patients have access to the resources and options they need to successfully progress through the Total Orthopedics Spine Care Program using his P-I-T-S philosophy of care (Pills – Injections – Therapy – Surgery), so they can live with less daily pain flares and an improved overall quality of life.

*Note: P-I-T-S acronym. Dr. Kechejian has developed a pain management assessment and treatment program to address America’s opioid crisis and pain epidemic and to help pain sufferers realize improved comfort and a better quality of life. For assessment of pain and quality of life, PITS stands for “P-physical function, I-intensity of pain, T-thoughts and behaviors, and S-social interactions”. These four areas of questioning cover a patient’s pain itself and how that pain is hurting their other aspects of life. Now for the treatment of pain, PITS also stands for “P-pills or medication choices, I-injection options, T-therapy choices, and S-surgery options, when needed”. These four areas of clinical care cover the basics of pain treatment options, starting with medications and injections to help break the pain cycle, then utilizing therapy techniques to help keep the pain from returning, and leaving surgery as a last resort if patients still remain in intractable pain, despite aggressive medical and interventional attempts at obtaining lasting comfort.
